Subscriber/Employee ID # or Social Security number: This information refers to the insured person and is not necessarily the patient. The Social Security number (SSN) is commonly used for computer and manual processing of claims.

The trigger for this information comes from the Insurance Plan screen, General tab, Patient ID to Use (Box 15) and comes from the Patient Information screen, Patient Information tab, Social Security Number or Other ID field. Note: The Box 15 reference is to the ADA 2003 form.
